I have read and consent to the Sutton Smart Business Card User Agreement below

By signing this form, I hereby;

Confirm that I currently have an office sharing agreement with one of the brokerages under Sutton Group Management Ltd. including but not limited to, Sutton Group West Coast Realty, Sutton Centre Realty and Sutton Group-1st West Realty.
Confirm that I have received my Sutton Smart Business Card (also referred to as, SSBC) in good condition and functioning order;
Agree to the security deposit charge for the SSBC in the amount of $125.00;
Understand that the SSBC incorporates two additional functions:
1. The ability to gain access to the brokerages under Sutton Group Management at the brokerage’s discretion;
2. Access printing services in the brokerage under Sutton Group Management at the brokerage's discretion;
Accept the responsibility and covenant to immediately report the loss or theft of my SSBC to office administration as soon as I become aware of it, should such an event occur and any liability for consequences of not reporting said loss or theft;
Understand that the fee to replace my SSBC in the event that it becomes damaged, lost or stolen is $125.00;
Agree to return my SSBC should I choose to no longer be engaged in an office sharing agreement with the brokerage under Sutton Group Management Ltd at the brokerage's discretion.
Acknowledge that all SSBC cards will remain the property of Sutton Group Management Ltd. and that Sutton Group Management, without notice and at its sole discretion, reserves the right to cancel this User Agreement and demand the return of the SSBC.
